*Please be sure to fill out this form as we will contact you if you are injured on the day.
(3) Please be sure to read the disclaimer
(4) Fill in the date and name of your attendance in the confirmation section (in the blank space at the top labeled "I" and in the "Name" section at the bottom).
*If the participant is under 18 years old, please be sure to sign the parent's name as well.
(5) Affix a seal or signature to the “seal”
(6) On the day of the class, please submit the completed form to the reception desk.
